Bernard Health helped Nashvillians in their healthcare decisions with a new plan that enables family members to make several tough decisions ahead of time.

Thursday night, Bernard Health in Nashville helped people come up with a plan that takes effect if they become unable to make your own health decisions.

Less than 10% of Tennesseans have one in place. The company is in the process of being certified to help families come up with their care plans.

"It's a hard but important conversation to have we've thought really deliberately about ways to talk to people and ways that we can make those decisions easier on them and family members and establish preferences ahead of time," said Ryan McCostlin.

It costs a flat rate of $99 to make the plan and people get to leave with it in their hand that day.
