Officials with the National Transportation Safety Board have released their preliminary report on a Collegedale plane crash that claimed three lives.

According to media outlets, the report did not state a primary reason for why the single-engine plane crashed on June 11 as it was coming in to the Collegedale airport.

Pilot Todd Silver was traveling with his mother, Suzanne Silver, and his two children. Todd Silver, Suzanne Silver, and a male child died in the crash. A 12-year-old girl was the only survivor.

A flight instructor at the airport told investigators that he saw the plane on a "short final approach to runway 3." No communication was reported between the airport and Silver's plane.

The NTSB report said there was no fuel in the right fuel tank. The left tank contained 15 gallons of fuel; however, it was seen leaking before the crash.
